The problems at the SQA represent the latest in a series of failures of organisations because of the introduction of new, expensive computer systems without proper risk assessment, without a proper trial period and without proper cognisance of the impact of the introduction of the new system on external organisations.
We also made a commitment to monitor the implementation of care in the community. Given that the new policy was started only on 1 July last year, it would have been petulant to start jumping up and down at the time, because anything so new was obviously going to have teething problems.
