It strikes me as a sad irony that, now that Bill Gates and Gordon Brown have left the room, instead of talking about how we drive forward strategic change in the NHS in Scotland, we are talking yet again about how we tinker with structures.The big question is how we drive forward strategic change, and it is a £10 billion question.

There are differences between Gordon Brown, Jack Straw and the Prime Minister on the issue, but I am delighted, if ministers have now read the fine print of the constitution and have seen the dangers, that they will sign up to if they adopt it.One of the main reasons for drawing up the new constitution was to allow the 10 new nations to enter.
