We found that not having problems with those four points was the best predictor of acceptable cleanliness, but that, in one or two hospitals, things were going wrong in spite of that.
We have come a long way since then, but I will follow up on a couple of points. One of them, which I mentioned on your previous visit, concerned the monitoring procedures.
