He changed solicitors"but his files were not passed on to his new solicitors. After some time, his original solicitors said that they had lost the files.
We recognise that the health service's data collection is not perfect and that there will be a problem if, as part of the move to the new contract, new needs are identified that cannot be funded quickly because of the move to a new capped system of funding.
