It assumes that 90 per cent of practices will achieve 90 per cent of the maximum amount of quality points. That is a fairly safe estimate and I guess that, despite their best efforts, practices will fall a bit short of that.
But the centre did not intervene to stop a debit building up that contributed to the financial problems at that point. Not in the days of the internal market—going back to the mid-1990s—when many fundholders sent patients to where they felt they would get the best service.
Is it possible that, instead of having recognised railheads or loading points for timber, you could operate using informal loading points in forests, particularly in the evening or through the night, when the lines will not be used for passenger transport?
