Health, Care and Sport Committee [Draft]
Meeting date: 4 November 2026

A commitment was made in last year’s budget to giving hospice staff pay parity with NHS staff. As you will know, there are particular challenges in the NHS Lothian area. Yesterday, I visited St Columba’s Hospice Care. Are you able to say a bit more about how you will ensure that that is implemented on the ground for the likes of St Columba’s Hospice Care for the next budget year? Outwith pay parity, will you say a bit more about the sustainable funding model?

I have a brief supplementary question. I welcome your comments on hospices. The other issue relating to what you said about the need to follow the money is about money going to local government in relation to prevention. Can you say more about how to follow that money through? I think that local authorities have had some challenges in terms of feeding that money through to where it should go.

My question is about the subnational arrangements, which Caroline Hiscox talked about when she was here. You have touched on this, but where do you see lessons from those arrangements being taken forward on the ground? In particular, what will be the budgetary impacts of the policy this year? We have heard that more operations will be carried out and there will be efficiencies. How do you see the subnational arrangements moving forward with the two health boards?

The Government’s infrastructure investment plan and the programme for government make reference to building new community and care hubs. Will you say a bit more about that? Those are incredibly important for local communities in bringing services back to them wherever possible.

The examples that you have given can provide lessons for the committee to learn from and observe as we go forward. As I said, we also previously heard evidence from Caroline Hiscox.

If we are talking about social care reform, a key point is that what we need to do will look different in different parts of Scotland, and in different parts of the UK, whether they are urban or rural. I was reading about the report that the University of Birmingham produced on asset-based approaches and on how you can look at having a local solution, which will be different depending on whether it is for the islands, the north of Scotland or urban Glasgow, for example.
If communities feel that they are invested in what is in their area, there is more of a connection, which is important. Will you say a bit more about the asset-based approach and where you can see it working in Scotland?

Yes. We have spoken about the need for investment in facilities and general development. Gregor Muir made a point about strategic return on investment and John Lunn touched on some of the work that has been done in that regard. Is there a need for broader work across Scottish sport to look at what the social return on investment should be if we are going to invest that £15.5 billion? If that work is required, who would do it? Would it be sportscotland, sports associations or the Government? There should be a way to quantify the return on investment over time. If we are going to invest that money, what would we be getting back? That goes back to Joe Long’s point about the budget. We need specific figures so that we can say what the return on investment will be. How do we tie that together?
That question is for Forbes Dunlop and then Gregor Muir.

Professor Knapp, in your submission, you talk about the social return on investment. If we are to move towards a preventative agenda, one of the key things is how long that will take and what we will achieve in that time. It is about truly moving from just trying to get by to having a preventative agenda. Professor Feeley said that we cannot afford not to invest in that, both for reasons of care and because it would not stack up financially—we would be spending more money. What are your thoughts on the social return on investment?

I will follow up on that. There are two issues for me: one is how we develop participatory sport in the community and the other is elite development.
Do one-year budgets and a lack of strategic planning have an impact? We are talking about how to improve elite-level performance and how to get more people involved in sport. Does the lack of certainty about longer periods impact on elite performance? Does it impact on community participation? Those are the fundamental questions.
