Chamber and committees

This year the Scottish Government has made £12m available to support the wellbeing of the pharmacist workforce. That includes £8m from the NHS Recovery Plan to provide ongoing wellbeing support for the workforce – alongside an extra £4m to provide further support to address winter pressures.

The funding supports the 24/7 National Wellbeing Helpline, our National Wellbeing Hub, Coaching for Wellbeing and the Workforce Specialist Service, alongside tailored psychological interventions for staff, providing individualised wellbeing support focusing on people’s physical and emotional needs.

The Chief Pharmaceutical Officer for Scotland in a joint letter, issued 8 December, with the leaders from the pharmacy sector, including the Royal Pharmaceutical Society (Scotland), has written to all pharmacy teams outlining the range of wellbeing support available to pharmacy teams and encouraging them to access these resources for support when needed.

We engaged with a range of stakeholders to inform the development of the draft Framework for Pain Management Service Delivery including the people with lived experience who were members of the National Advisory Committee for Chronic Pain (NACCP).

The structure and membership of the Committee was reviewed in 2020 in order to increase representation of people with lived experience and other key groups with an interest in improving care and support for people with chronic pain. As set out in the updated Terms of Reference, the Committee is advisory in nature, and members were encouraged to seek consensus in providing advice. The Committee met in its new format in February and May 2021 with all members invited to attend these meetings. Members were also asked to provide feedback and responses a number of other relevant request between meetings.

All members of the Committee had the opportunity to freely share their views on the priorities for improving pain management support which were used to inform the development of the draft Framework. Lived experience members of the Committee were provided with additional support in their role in a number of ways including: meetings to gather the views of a wider cohort of people with chronic pain via the Chronic Pain Patient Reference Group; being offered the choice to participate in capacity building sessions; and being offered the opportunity to directly share a summary of the views of people with chronic pain both in advance with officials and during Committee meetings as part of dedicated agenda items. We have also carried out a Scotland-wide survey to better understand the priorities of people with chronic pain and increase the range of voices involved.

All members of the Committee had access to the same information and resources throughout the development of the Framework, and were provided with a draft version in advance of the launch of the public consultation.

As communicated to the Committee previously, we are currently reviewing existing stakeholder engagement processes and structures, including the NACCP, to ensure that they are fit for purpose and can meaningfully support the future delivery and implementation of the Framework once the current public consultation has been completed.

An aim of the refreshed Out-of-Hospital Strategy 2021 to 2026 is to ensure that 20% of all OHCAs will have a defibrillator applied before the arrival of ambulance service. We want to improve bystander confidence in the use of these devices and the Strategy includes an aim for the Save a Life for Scotland partnership to increase familiarisation with defibrillator usage across Scotland, through embedding defibrillator usage in awareness raising campaigns and CPR training.

The Scottish Government continues to work with NHS National Education for Scotland (NES) and the Royal Pharmaceutical Society in developing a joint programme to encourage more students, and not just school leavers, to consider careers in pharmacy, including ensuring the output reaches deprived and remote and rural communities.

In addition, both Schools of Pharmacy work towards the widening access targets set by the Commission on Widening Access for universities to drive forward the delivery of equal access in Scotland.

There are also wider initiatives such as The Scottish Wider Access Programme (SWAP). The partnership develops access programmes for adults returning to education, and targets adults who reside in the most disadvantaged areas of Scotland and particularly those who have no, few or out of date qualifications. This can help support people with few suitable qualifications to progress to degree level Pharmacy studies.

As you are aware, our NHS is currently facing the most difficult circumstances in its history due to the ongoing COVID-19 pandemic. This is exacerbated at present due to the potential risk posed by the Omicron variant to our healthcare system. This means Health Boards are having to make very difficult decisions about prioritising care, and I know this has an impact on everyone who is waiting for support for their health at this time.

We recognise that this includes people experiencing chronic pain, and that is why we are providing funding to enhance the capacity of pain management care through the Chronic Pain Winter Support Fund. We will shortly announce the outcome of this funding call. The projects we fund will boost the support available for people with chronic pain at the national and local level to help them maintain their health and wellbeing over the months ahead.

There are approximately 240 students each year who graduate from the two Schools of Pharmacy in Scotland. There has been an increase in the overall numbers of students studying pharmacy with more Scottish domiciled students undertaking the course. The two Schools of Pharmacy in Scotland are encouraging applicants from parts of Scotland where there are gaps in the pharmacy workforce and working with schools to promote pharmacy as a career.

Since 2018, the number of NHS pre-registration pharmacists places funded nationally has increased from 170 in 2018 up to 231 in 2020-21. In 2021-22, funding of £6,096,489 was provided to NHS Education in Scotland (NES) for the Pre-registration Pharmacy Scheme, for 231 places. By 2024-25 this will have increased by a further 89 places to bring the total to an additional 120 pharmacist trainees over 5 years staying in Scotland to complete their pre-registration training.

In order to meet future workforce demands, a five year integrated initial education programme for pharmacists in Scotland has been developed. The first students to graduate from the new programme will qualify in 2027 with co-terminus graduation and registration. This will better prepare newly qualified pharmacists for practice, strengthen the clinical workforce, particularly in primary care, and manage trainee numbers to meet workforce requirements.

The Scottish Government is committed to having a pharmacist independent prescriber in every community pharmacy. As of December 2020 there were a total of 1585 pharmacists qualified prescribers or currently undertaking an independent prescribing qualification. As demand for pharmacy technicians grow and a skill mix is increasingly required in different pharmacy settings, increasing the pipeline of pharmacy technicians has become a priority. We are currently working with key stakeholders to develop a national pharmacy technician apprenticeship scheme.

As part of the overall 2021-22 community pharmacy financial package, a total of £2.751m provides each community pharmacy contractor with a monthly contribution of £180 towards staff training beyond pharmacists and pharmacy technicians.

We welcomed the launch of the Circuit, which aims to improve access to defibrillators across the UK. We would urge all defibrillator guardians to register their defibrillator so that the Scottish Ambulance Service call handlers can direct people to them in incidents of cardiac arrest. Registration of defibrillators in Scotland will build the evidence to support people and organisations in making informed decisions about where to place their life saving device.

The Scottish Government does not hold information on temporary closures of community pharmacies or the possible cost incurred by the NHS.

The provision of community pharmacy pharmaceutical care services are the responsibility of territorial Health Boards as set out in the National Health Service (Pharmaceutical Services) (Scotland) Regulations 2009. Any challenges faced by community pharmacies in ensuring the safe and effective dispensing of treatments should be discussed with the local Health Board.

The Centre for Sustainable Delivery (CfSD) (and prior to this the Modernising Patient Pathways Programme within Scottish Government), have supported the rollout of the Digital Dermatology Appointment (DDA) system. This platform allows patients to interact with dermatologists by answering a series of questions and uploading images of their skin condition for the clinician to review. This system is used for both new and return patients. The system was offered for rollout to all Health Boards during COVID and to date is live in five Health Boards, with over 4000 patients having appointments this way.

In addition, the CfSD is actively promoting Active Clinical Referral Triage in Dermatology and encouraging the use of images to support this where this can be done safely. We are currently engaged in a piece of work to understand the use of images in referral pathways across all Scottish Health Boards and the output of this will support key work in 2022 to increase the use of images to support timely access to Dermatology care.
